How to add a new survey questionnaire/package step by step?

Go to content management, then go to the Surveys tab.


Click on “Add new survey package”. Enter your desired package name, select or upload an image (this image will appear to the participant on the My Task page) & click on next.


Add your page(s) in the “Add Page” tab.

The page description will be displayed below the name of the page to the participant/ user.
The number of pages you add here is how it will be displayed to the participant. In later steps we will map questions to pages, allowing participants to see specific statements on each page.

a. All statements on one page: You can add one page & later map all statements on this page. The user will then see all questions on one single page.

b. Multiple pages for multiple sections: Add pages such as “Section 1”, “Section 2” and so on, you can later map multiple questions to each of your pages.

Once you have added your pages, click “Save & Next”.

Add your questions in the “Add Questionnaire” tab.

Based on the number of pages you have added you will be able to see one or multiple sections accordingly

  1. To add a question select the type of question it is going to be, Likert or Text.

  2. Enter the question. 

  3. Select a scale (you can either select a default scale from the scale library or enter your own scale)

  4. Click on "Save and Add Question" to add the question on the same page or click on "Save"


For likert questions, participants will see the scale options below the statement when filling the survey. For text questions, participants will see a text box area where they can type in their responses.


Configure Survey Settings

  1. Select if the survey is optional or mandatory for the user.

  2. If the survey is optional, users can submit it without answering any questions.

  3. If the survey is mandatory users have to answer every single question before submitting the survey
    Once done, click on save. You can now go ahead & deploy your survey.
